Passport Service at the Registry of Deeds

The Middlesex North Registry of Deeds now accepts passport applications on behalf of the U.S. Department of State.  We accept applications Monday through Friday from 8:30 a.m. until 4:00 p.m.

Due to the complexity of the passport application, please complete the form online, print it and bring it with you to the Registry.  The forms and additional information are available at the State Department's website.  (We do have forms available here at the Registry if needed).

In addition to the completed forms, you must also bring:

  • two (2) passport photos
  • two (2) blank checks (one for the State Department and one for the Registry)
  • evidence of citizenship (usually a birth certificate or Naturalization papers)
  • a picture ID
